The Health Examination Form to be completed and submitted prior to entry into first grade.

In order for your scholar to receive medication on campus, JAA must receive this form from the physician each year.

To be completed by oral health professional and submitted by May 30th of the Kindergarten year.

For completion by scholars in grades 7-12. Please return to Advisory Teacher.

Please complete these forms if your scholar has a history of seizures or has been diagnosed with epilepsy.
